Elisabeth Sparkle (Demi Moore), a former A-list celebrity whose best years appear to be behind her, is abruptly fired from her television fitness show by ruthless network executive Harvey (Dennis Quaid). She soon discovers a mysterious drug known as The Substance, which promises the chance to become a younger, more perfect version of herself.
One injection produces Sue (Margaret Qualley), a beautiful younger version of Elisabeth—but there is one strict rule: the two bodies must share their time equally, switching every seven days without exception. As that balance begins to break down, the consequences become increasingly disturbing.
Written and directed by Coralie Fargeat, The Substance is a provocative body-horror film starring Demi Moore, Margaret Qualley, and Dennis Quaid. The film won the 2025 Academy Award for Best Makeup and Hairstyling and received five Academy Award nominations.
